Well, I have an A4 Intuos3 (9x12). It's a great tablet, but due to it's size it's not very mobile, so I decided to get a small Intuos4.
It came today, and all I can say is wow. O.o The pen sensitivity is noticeably better. Seriously. I think it also has a larger surface resolution, but whatever it is, it makes much crisper and smoother strokes.
A feature I especially like is that with this version you can set the response level of the pen; whether you want it to respond to firm or soft preassure, a feature that was absent with Intuos3. The ambidexterous design is also a nice feature. Speaking as a lefty, I like that I don't have to sit there accidentally hitting off buttons that would otherwise be in the way.
My only problem with the Intuos4 is a pretty petty one. Photoshop doens't seem to recognise the pen eraser. It did when I installed it first, but now it won't, and despite multiple reinstalls, I still can't seem to get it to work. It recognises the Eraser in painter though, so I know it works. I just don't know what I did.
All I can say is, if you are a tablet user, get an intuos4, because it is SO worth it. Like I am not even kidding. I mean I love my Intuos3 for it's size, but I may not use it anymore the way things are looking...

I'm using a Wacom Volito2 A5 since some years, and working with it is quite okay. But it's more an Office Graphic Tablet, that's why I want to buy an Intuos, and because I think Intuos is way better
Small and medium sizes are good for those with limited space or those who need portibility. But I mean, in the end, the only difference between all of them is active area size, so it's really up to you
It's good that you're thinking of getting one. It really is a great tablet.
